Form for each person who plans to attend the conference
(parent + daughter = 2 forms). |

| Step 2 - Important! - Confirm that registration is still open |

Visit Registration to confirm that registration is still open. We limit enrollment to the first 450 girls who apply, and we do not have a waiting list. |

| Step 3 - Pay $6 per person |
| |
Click Add to Cart (if necessary, disable your pop-up blocker). To pay for more than one registration fee, simply change the quantity (Qty) when you reach the PayPal web site.
Note You do not need a PayPal account to use this service.
Note If you see PayPal's Shipping Information page, check the first button, "Shipping is not required," and then click the Continue Checkout button. |

| Step 4 - Print your PayPal confirmation page |

When the payment confirmation page appears, print one copy for us and, if you like, one copy for your records. (This is the PayPal web page titled "You Have Completed Your Purchase.")
Please legibly write the applicant's name or Group Leader's name on your PayPal confirmation page, in case it gets separated from your registration form(s).
Note After "You Have Completed Your Purchase," we will receive an email message telling us your name, email address, the amount you paid, and your receipt number. We do not receive your credit card information. |

| Step 5 - Mail forms to us |

Mail your completed registration form(s) and payment confirmation(s) to the address below. (Group Leaders, please include a roster of all adults and students in your group.)

| Step 6 - Watch your email - *NO postcards will be sent this year |

If we have your correct email address, you should receive a notification confirming your registration. If you don’t receive the email by February 27, parents or teachers may contact us at eyhhouston@gmail.com. Please include the registered person’s name, school, and email address. |

If paying by check, please include a check for $6 per person made payable to West Harris County AAUW/EYH and mail to the address above. Please mail your registration form(s) and payment together. We cannot accept payments without registration forms or registration forms without payment. |
